The circulatory system is a system of organs that includes the heart, blood vessels, and blood which is circulated throughout the entire body of a human.

It includes the cardiovascular system, or vascular system, that consists of the heart and blood vessels.

It consists of the following organs and tissues:

What is Heart ?

A muscular pump that forces blood around the body. Heart is a muscular organ covered with three layers.

  • Epicardium 
  • Myocardium ( Middle layer)
  • Endocardium 

 

1. A closed system of blood vessels: These vessels include:

  • Arteries: Vessels that carry blood away from the heart.
  • Veins: Vessels that bring blood back to the heart.
  • Capillaries: Tiny vessels that branch off from arteries to deliver blood to all body tissues.

Pacemaker 

A pacemaker is a small device that’s placed in the chest or abdomen to help control abnormal heart rhythms. The device uses electrical pulses to prompt the heart to beat at normal rate.

  • It consists of leads means electrode and pulse oxygenator means battery.
  • Check single chamber pacemaker, Dual chamber pacemaker, multiple chamber pacemaker.
  • Position of electrodes: Temporary vs permanent, epicardial vs Transvenous.
  • Check: placement, insertion, removal, replacement, upgradation.
  • Replacement of Device : Removal of battery, Removal of leads, insertion of the new system device.

  • Temporary/ permanent 
  • Procedure 

 

Implantable defibrillator: 

Defibrillator are devices that restore a normal heartbeat by sending a electric pulse or shock to the heart. They are used to prevent or correct an arrhythmia, a heartbeat that is uneven or that is too slow or too fast .

Defibrillator can also restore the heart’s beating if the heart suddenly stops.

Two types of implantable defibrillator 

  1. Transvenous implantable cardiverter defibrillator 
  2. Subcutaneous implantable defibrillator .

Positioning of electrodes: 

  • Transvenously
  • Epicardially
  • Subcutaneous 

 

Check leads : single lead, dual lead, multiple lead.

1. CABG( Coronary artery bypass graft):

In CABG topic what type of graft is harvest in scenario like: venous graft, arterial graft, combined graft.

2. Venous graft code range(33510 – 33516)

Don’t code venous graft codes (33510-33516) with 33533 to 33536 ( arterial graft).

3. Combined graft code range ( 33517-33522)

We can code 33517 with 33533 to 33536.

4. For arterial graft code (33533 – 33536)

Note: If CABG scenerio harvesting is given saphenous vein, mammary and low extremity artery then no need to separate code.

If CABG scenerio harvesting is given femopopliteal vein then need to separate code (35572).

 

  • Endoscopic code 33508 we can code with 33510-33523. Don’t add any modifier with add on codes.

 

Catheter placement Topic:

  • Check for veins given in scenario
  • Central vein : you will find one key term like Subclavin, juglar, femoral vein, Inferior vena cava.
  • Peripheral vein: Basilic, Cephalic, Sephanous.

Note: Tunneled means Chest incision and tunnel creation.

Non-tunneled means direct puncture of the Central vein.

  • Check : Tunneled or non Tunneled.
  • Check with or without port pump.
  • Then check patient age.

 

5.If insertion and reposition on same day then code only insertion don’t code reposition.

 

ECMO Guidelines CPT

Extracorporeal membrane oxygenation (ECMO) or Extra corporeal life support system (ECLS):

  • It provide cardiac and respiratory support to the heart and or lungs which allow them to rest.
  • Two methods 
  1. Venoarterial
  2. Venovenous

Cardiac Assist

  • Ventricular assist device – VAD
  • Removal of VAD includes removal of entire device including cannulas.
  • Removal of percutaneous VAD at the same session as insertion is not separately reportable.
  • Removal of percutaneous VAD at the separate and distinct session but on same day as insertion report 33992 and append modifier 59.

 

Endovascular repair of Abdomen Aorta or iliac arteries:

  • Endovascular repair of abdominal aortic aneurysm 34800-34834.
  • Endovascular repair of iliac aneurysm 34900.
  • Decompressive laparotomy for abdominal compartment syndrome, after ruptures abdominal aorta or iliac aneurysm may be separately reported with 34900 in addition 34702, 34704,34706 and 34708.

Vascular injection procedures:

  • Intravenous and intraarterial 
  • Both have nonselective and selective.
  • Nonselective catheterization – catheter tips is placed in major blood vessels 
  • Selective catheterization – catheter tips other than major blood vessel.

 

Endovascular revascularization:

  • Three territories:1.  Iliac 2. Femoral/popliteal 3. Tibial/ peroneal.
  • Code arrange for each hierarchy 
  1. Stent placement with atherctomy 
  2. Atherctomy 
  3. Stent placement 
  4. Angioplasty 
  • Iliac vascular territory don’t include atherctomy.
