What is Urinary System ? 

The urinary system’s function is to filter blood and create urine as a waste by-product. The organs of the urinary system include the kidneys, renal pelvis, ureters, bladder and urethra.

Types Of Urinary System:

  • Kidney 

 

The kidney and urinary systems help the body to eliminate liquid waste called urea, and to keep chemicals, such as potassium and sodium, and water in balance.

 

Two kidneys

This pair of purplish-brown organs is located below the ribs toward the middle of the back. Their function is to:

  • Remove waste products and drugs from the body

 

  • Balance the body’s fluids

 

  • Release hormones to regulate blood pressure

 

  • Control production of red blood cells

 

Two ureters 

These narrow tubes carry urine from the kidneys to the bladder. Muscles in the ureter walls continually tighten and relax forcing urine downward, away from the kidneys.

About every 10 to 15 seconds, small amounts of urine are emptied into the bladder from the ureters.

Bladder

This triangle-shaped, hollow organ is located in the lower abdomen. It is held in place by ligaments that are attached to other organs and the pelvic bones. The bladder’s walls relax and expand to store urine, and contract and flatten to empty urine through the urethra. 

 

Urethra

This tube allows urine to pass outside the body. The brain signals the bladder muscles to tighten, which squeezes urine out of the bladder.

  1. We have to check in the scenario what type of procedure like open, laproscopic, endoscopy.

 

For Nephrectomy code in Genitourinary System CPT Guideline : 

Look –  Open / laproscopic.

Total, partial/ radical nephrectomy.

50220 – Code for Nephrectomy 

50240 – For partial Nephrectomy 

50543 – For laproscopic nephrectomy.

Stone removal percutaneous: Lithotripsy can be used with stone basket extraction – 50080.

Percutaneous Lithotripsy – 50590.

 

52000 vs 52005 code 

52000 – we have to code when cystoscope reach to urethra.

52005 – we have to code when cystoscope reach to the Ureter.

 

Renal Transplantation: 

Renal autotransplantation includes reimplantation of the autograft as the primary procedure, along with secondary extra- corporeal procedures (e.g partial Nephrectomy, nephrolithotomy) reported with modifier 51 (50380 and applicable secondary procedure).

Renal allotransplantation involves three distinct components of physician work: 

  • Cadaver donor nephrectomy, unilateral or bilateral 
  • Backbench work
  • Recipient renal allotransplantation : which includes transplantation of the allograft (with or without recipient nephrectomy and care of the recipient (50360,50365).
  1. To report cystourethroscopic removal of a self retaining, indwelling ureteral stent 52310, 52315 and appen modifier 58 , if appropriate.

 

  Difference between CPT 52310 and 52315 

  • 52310 : Cystourethroscopy , with removal of foreign body, calculus or ureteral stent for urethra or bladder; simple.
  • 52315 : Cystourethroscopy , with removal of foreign body, calculus or ureteral stent for urethra or bladder; Complicated.

 

Anatomy of Male Genital System 

What is male Genital System? 

Most of the male reproductive system is on the outside of your abdominal cavity or pelvis. The external body parts of the male reproductive system include the penis, scrotum and testicles. Another name for these parts is genitals or genitalia.

Penis : The penis is the male organ for sexual intercourse. It contains many sensitive nerve endings, and it has three parts:

  1. Root. The root is the base of your penis. It attaches to the wall of your abdomen.
  2. Body (shaft). The body has a shape like a tube or cylinder. It consists of three internal chambers: the two larger chambers are the corpora cavernosa, and the third chamber is the corpus spongiosum. 
  3. Glans (head). The glans is the cone-shaped tip of the penis. A loose layer of skin (foreskin) covers the glans. Healthcare providers sometimes surgically remove the foreskin (circumcision).

Scrotum: 

The scrotum is the loose, pouch-like sac of skin that hangs behind the penis. It holds the testicles (testes) as well as nerves and blood vessels.

Testicles

The testicles (testes) are oval-shaped organs that lie in your scrotum. They’re about the size of two large olives. The spermatic cord holds the testicles in place and supplies them with blood.

Epididymis

The epididymis is a long, coiled tube that rests on the back of each testicle. It carries and stores the sperm cells that your testicles create. 

Vas deferens

The vas deferens is a long, muscular tube that travels from the epididymis into the pelvic cavity, just behind the urinary bladder. The vas deferens transports mature sperm to the urethra in preparation for ejaculation.

Urethra

The urethra is the tube that carries pee from your bladder outside of your body. If you have a penis, it also ejaculates semen when you reach orgasm.

Seminal vesicles

The seminal vesicles are sac-like pouches that attach to the vas deferens near the base of the bladder. Seminal vesicles make up to 80% of your ejaculatory fluid, including fructose. Fructose is an energy source for sperm and helps them move (motility).

Code 54150 vs 54160 we need to check with clamp aur without clamp

  • When clamp + ring block/ dorsal penile is used then we have to code 54150.
  • When clamp is not used then we have to code 54160.

 

54163 – Repair incomplete circumcision/ repeat circumcision.

Note : use 52 modifier with code 54150 when procedure done without ring block.

 

Prostate biopsy: 55700, 55705, 55706.

What is Female Genital System? 

The female reproductive system consists of internal and external organs. It creates hormones and is responsible for fertility, menstruation and sexual activity.

 

Parts of Female Genital System 

External parts

The function of your external genitals are to protect the internal parts from infection and allow sperm to enter your vagina.

The main parts of your vulva or external genitals are:

Labia majora: Your labia majora (“large lips”) enclose and protect the other external reproductive organs. 

Labia minora: Your labia minora (“small lips”) can have a variety of sizes and shapes. They lie just inside your labia majora, and surround the opening to your vagina (the canal that joins the lower part of your uterus to the outside of your body) and urethra (the tube that carries pee from your bladder to the outside of your body). 

Clitoris: Your two labia minora meet at your clitoris, a small, sensitive protrusion that’s comparable to a penis in men or people assigned male at birth (AMAB).

Hymen: Your hymen is a piece of tissue covering or surrounding part of your vaginal opening. It’s formed during development and present during birth.

Internal parts

Vagina: Your vagina is a muscular canal that joins the cervix (the lower part of uterus) to the outside of the body. It can widen to accommodate a baby during delivery and then shrink back to hold something narrow like a tampon. 

Cervix: Your cervix is the lowest part of your uterus. A hole in the middle allows sperm to enter and menstrual blood to exit. Your cervix opens (dilates) to allow a baby to come out during a vaginal childbirth. 

Uterus: Your uterus is a hollow, pear-shaped organ that holds a fetus during pregnancy

Ovaries: Ovaries are small, oval-shaped glands that are located on either side of your uterus. Your ovaries produce eggs and hormones.

Fallopian tube: These are narrow tubes that are attached to the upper part of your uterus and serve as pathways for your egg (ovum) to travel from your ovaries to your uterus.

Vulvectomy procedure: 

Check simple vs radial

Partial vs complete 

Note : simple – Removal of skin and superficial subcutaneous tissue.

Radial – Removal of skin and deep subcutaneous tissue.

Partial – Removal of skin and less than 80% vulvar area.

Complete – Removal of skin and greater than 80% vulvar area.

Vulvar area code range (56620 – 56640).

Colporrhaphy : cystocele, Rectocele, enterocele repair.

Endoscopy on cervix uteri 

 

  • Biopsy + ECC then code is 57454

 

  • Only biopsy then code is 57455.

 

  • Open biopsy then 57505.

 

  • Only ECC is then code 57456.

 

  • Loop electrode biopsy: 57460.

 

  • Conization : 57461 , conization open procedure then code is 57520.

 

  • Conization + ECC both then code is 57456.

 

Hysterectomy:
